Another particularly interesting case was successfully completed in our clinic using the endoscopic method for the spine. Our clinic is recognized as a center of excellence and a global training center for endoscopic and minimally invasive spine techniques. It is worth noting that endoscopic surgery for the cervical spine was first introduced in Greece by our clinic and Dr. Georgios Vastardis, MD, PhD, seven years ago. A 48-year-old patient presented with severe pain in the right upper limb, accompanied by neurological deficit in the right triceps muscle (strength 4-/5). Imaging revealed a right foraminal disc herniation at the C6-C7 level, causing right-sided radiculopathy involving the C7 nerve root. The patient underwent posterior endoscopic discectomy at C6-C7 on the right side and was discharged home just three hours after the procedure, completely pain-free. The strength in the right triceps muscle significantly improved (4+/5). On the same day, he began physiotherapy to address the remaining neurological deficit. Minimally invasive techniques continue to deliver outstanding results, ensuring faster recovery and an improved quality of life for our patients! Off-Center Imaging in MRI: Challenges & Adjustment Strategies In the ideal MRI world, we always strive to center the region of interest (ROI) at the magnet's isocenter. However, in daily practice, off-center imaging is sometimes unavoidable – especially in cases involving large fields of view, trauma patients, pediatric imaging, or orthopedic exams. Challenges of Off-Center Imaging: Image distortion due to gradient non-linearity and B0 inhomogeneity. Reduced signal-to-noise ratio (SNR) away from the isocenter. Geometric inaccuracies affecting diagnostic interpretation. Fat suppression failure, especially in off-axis regions. 🔬 Dual-Source Photon-Counting CT for Morphological and Functional Assessment of Myocardial Bridging The evaluation of myocardial bridging (MB) has traditionally relied on morphological imaging, often underestimating its dynamic systolic behavior and potential hemodynamic impact. With the advent of dual-source Photon-Counting CT (PCCT), both morphological and functional characterizationof MB can now be approached within a single non-invasive acquisition. Key enabling features include: ⚙️ Ultra-high spatial resolution (0.1 mm) for accurate delineation of the intramyocardial segment and perivascular tissue interface. ⚡ High temporal resolution (~66 ms) through dual-source acquisition, allowing visualization of systolic compression (“milking”) and diastolic decompression across the cardiac cycle. 🌈 Spectral quantification capabilities, enabling iodine mapping and potentially dynamic perfusion analysis, to correlate luminal narrowing with downstream myocardial enhancement patterns.
