The Shifting Responsibility of Medical Equipment in Malaysia: Past, Present, and What’s Next? Medical equipment forms the backbone of healthcare — from basic beds to robotic-assisted systems. Over the last two decades, responsibility for maintaining this equipment in Malaysian hospitals has shifted significantly, offering both benefits and drawbacks. The Early Days: Vendor-Led Support Before I graduated in 2008, vendors typically maintained the equipment they sold. With few biomedical engineers around, vendors were key to ensuring uptime. Good service meant repeat orders. My first employer in Sabah built a reputation this way — starting with hospital linens and eventually supplying everything from beds to lab machines. The Rise of Concessionaires In the early 2000s, the government outsourced hospital maintenance to a few major concessionaires. The aim was to reduce reliance on vendors’ limited manpower and speed up response times through in-hospital engineers. While efficient, this model had a downside — engineers couldn’t master every machine type. A trade-off between efficiency of service and quality of service. Post-COVID Shift: Leasing Programs More recently, the Malaysian government adopted a time tested scheme, leasing program, either Lease-to-Own (equipment stays) or Lease-to-Use (equipment returns to vendor). Leasing helps spread costs over 1–5 years and shifts maintenance back to the vendor. However, it often inflates costs by 20–30%, as vendors cover longer warranty periods and repair risks. The end user loves this program, since they will be able to change newer technology after the leasing period, but not so much to the vendor. This program favors financially stronger companies, over smaller financially-strapped vendors. Lease-to-use adds another issue: disposal. With little market for used medical equipment, returning machines after five years is inefficient, especially when their lifespan is 7–10 years. A 10-year lease seems logical, but few vendors can afford to tie up capital that long. In business, cash flow is king. Tying up your capital for 10 years is a financial nightmare for most CFO, or small scale business owner. So, what’s Next for Malaysia healthcare? Maybe, Pay-per-Use? In this model, vendors retain ownership and earn based on how often the equipment is used. It aligns incentives — vendors maintain uptime, and hospitals pay only when machines are used. This could reduce waste, extend equipment life, and promote accountability. Most importantly, vendor will not supply substandard equipment since a longer lifespan translate to better profit to them. So we could weed out the inferior quality device in our hospitals. But this may raise healthcare costs, putting more financial burden on the public. Then again, one could argue it discourages overuse and aligns usage with actual need. Perhaps it’s time to shift focus from ownership to optimized usage. Do you think this is workable?
