We Must Evolve Last week, I had the privilege of speaking with our incredible CKD and ESRD care teams, physicians, and leaders at Balboa Nephrology, Fresenius Medical Care, and our new partner Champion Health Plan about something that transcends medicine — the human side of healing. We often talk about data, labs, and outcomes. But behind every patient is a story shaped by the social determinants of health — housing, food, transportation, trust, and belonging. These are often the true barriers to wellness. I shared five reflections that continue to guide our work in Southern California: 1. Recognize that a patient’s social situation—not just their labs—can be the biggest barrier to health. 2. Reflect on how empathy and connection sustain our own well-being. 3. Reframe our mission: we don’t just treat kidneys; we treat the circumstances that make kidneys fail. 4. Reconnect with purpose and partnership—because when we connect, there is mutual healing. 5. Recommit to closing the gap between what we wish we could do and what patients truly need. The literature shows that while caring for patients with multiple social determinants of health increase the risk of burnout, it is advocacy that restores meaning and resilience for our physicians and care teams. When we stand up for our patients, we find purpose again. We will continue to move from organ-centered medicine to whole-person care, from compliance to co-creation, and from burnout to meaning. Archbishop Desmond Tutu once said: “There comes a point where we need to stop just pulling people out of the river. We need to go upstream and find out why they’re falling in.” It’s time to go upstream.
