PBM reform is finally within reach – at least that is the hope. What exactly should reform look like? For years, the largest PBMs have exerted a seemingly unbreakable monopoly in the pharmacy care industry, controlling more than 80% of the market. These legacy PBMs prioritize profits over patients, inflating prescription drug costs with no associated improvement in health outcomes. The business practices of the traditional PBM companies have also contributed to an alarming number of community pharmacies either going bankrupt or closing altogether, creating pharmacy “deserts” in an estimated 45% of US counties.
