About us

What is Smart Living? It is a new way of delivering services, using a value based tech-first approach. It includes new ways to assess staffing, the needs of the individuals, the way to delivery services, and the cultural methodology of inclusive services. A Smart Living agency works to provide value-added services as much as possible providing a means to provide services to more individuals in a sustainable manner. Smart Living is not an initiative, or a time limited project, it is the new model for service delivery. Smart Living is not one size fits all, each agency will do it differently, it is taking what works for the agency and building on it.

  • Florida Community Care (FCC), the Medicaid MCO serving people with intellectual and developmental disabilities (I/DD), enrolled 323 new members as Florida expanded its Intellectual and Developmental Disabilities Comprehensive Managed Care (ICMC) Program from a pilot to a statewide model across all 67 counties. Total enrollment has now reached 700 individuals. The ICMC Program brings together health care services, home- and community-based supports, and case management under one coordinated structure. Enrollees work with FCC case managers to identify needs and develop individualized plans. Covered services include applied behavior analysis, life skills development, personal supports, transportation, and residential supports such as group homes and supported living. The expansion has also created new pathways for individuals waiting for services. As of May 2025, 358 people on Florida’s iBudget waiting list have enrolled in ICMC. APD currently serves 35,790 individuals, with more than 21,000 still on the waiting list. #Medicaid  #Inclusion #SmartLiving #CommunitySupports #AssistiveTech

    Last week, the ShiftAbility team hit the road to Carroll, Iowa, to complete the first technology installation in New Hope‘s newest program! Too often, people assume tech-enabled supports are only for large cities; but we are proving otherwise. With ShiftAbility’s Smart Living model, we’re showing that transformative support can happen anywhere— rural or urban. The individuals were excited to explore the new tools in their home and begin their journey toward greater independence, safety, and choice.     Unity House is excited to launch a new Smart Living pilot program! We’re partnering with ShiftAbility, a consulting company that helps organizations create better, more flexible supports for people with disabilities through smart technology and remote support. Smart Living uses everyday technology to help people live more independently, safely, and confidently. This includes tools like: -Wearable devices (smart watches, smart rings) -Health tracking tools -Personal hygiene and daily task reminders -Fall detectors and motion sensors -Smart appliances with automatic shut-offs In a Smart Living home, technology can support people in many ways: from a fridge that tracks what’s inside, to sensors that can detect emergencies, to devices that allow someone to video-chat for help whenever they need it. These tools can make daily life easier, support people in the community, and provide help without taking away independence. During this pilot program, residents will work with our staff members to create a customized, technology-enhanced support plan designed around their goals and what works best for them. We’ll be sharing more soon as ShiftAbility’s system gets fully set up, but we’re excited about the new possibilities this brings and the chance for even greater independence, inclusion, and choice for the people we support.   • Across the country, states are rethinking how systems work together to support individuals with developmental disabilities as they age — and the families who support them. Sixteen states and the District of Columbia have joined a five-year initiative, the Bridging Aging and Disability Community of Practice, led by the Administration for Community Living (ACL). The goal: to strengthen connections between aging and disability networks, improve caregiver supports, and plan for the future of adults with I/DD. With over 7 million individuals living with intellectual and developmental disabilities — many cared for by aging family members — this collaboration couldn’t come at a more important time.
