We’re proud to share that Snohomish County’s Community Navigator Pilot Program officially wrapped up at the end of June 2025 after six months of building bridges between public health and historically excluded communities. Through partnerships with 12 incredible community-based organizations, the program trained and supported 64 navigators to deliver culturally responsive outreach, co-create health events and foster trust. Initial evaluation shows powerful impacts and valuable lessons for future equity efforts.
