As a nurse in Oregon, my purpose is advocacy — for my patients, my colleagues, and the future of healthcare. Lately, I keeping seeing the message from hospital systems: Healthcare is becoming more expensive, and the rising age and acuity of our population are making it harder to deliver care sustainably. I agree. The facts back it up: Oregon’s population over age 65 grew by over 50% between 2010 and 2020 (U.S. Census Bureau). Hospital expenses have risen, especially in labor, drug costs, and technology. Preventative healthcare is faltering — a Kaiser Family Foundation report found Oregon saw a 20% drop in preventive services during COVID-19, worsening chronic conditions. Our system is no longer designed this way. I am not a business expert, nor do I claim to have a degree in finance. What I do know is bedside care. And from my perspective, it is clear: under the current business model, many hospital systems are not sustainable. But the solution cannot simply be demanding more from patients, insurers, or the government. It must start within. Top-heavy compensation structures, multimillion-dollar executive salaries, and bloated bonus systems cannot coexist with worsening direct care staffing, skyrocketing turnover, and burned-out nurses. When frontline nurses advocate for safe staffing, we are not the problem. CNOs (who never touch a patient) making $10+ million a year while bedside nurses are asked to "do more with less" is not sustainable. This is the problem. I believe in the potential for hospitals to lead meaningful change — but it will take accountability, humility, forethought and a realignment of priorities back to where they belong: the patient bedside.

The Oncology Nursing Society (ONS) has released a pivotal white paper titled "The Impact of Climate Change Across the Cancer Control Continuum: Key Considerations for Oncology Nurses." This comprehensive document underscores the profound effects of climate change on cancer care, emphasizing the critical role oncology nurses play in addressing these challenges. As we step into 2025, it's imperative to recognize that environmental health is intrinsically linked to cancer care outcomes. Climate-related emergencies can disrupt cancer care delivery, from prevention and diagnosis to treatment and survivorship. Moreover, environmental exposures contribute to cancer incidence and can exacerbate health disparities, particularly in vulnerable communities. ONS's 2024–2027 Research Priorities highlight the necessity of understanding environmental impacts on cancer care (Rosenzweig et al., 2024). By focusing on the intersection of environmental health and oncology, we can develop strategies to mitigate risks, ensure seamless care delivery during climate-related disruptions, and advocate for policies that promote environmental justice.
