🚨 Medicare Update: Government Shutdown Impact 🚨 With the 2025 federal government shutdown now underway, CMS has issued several important updates that affect providers: 🔹 Claims Hold- Medicare Administrative Contractors (MACs) have been directed to hold certain claims for up to 10 business days. This precaution helps avoid reprocessing if Congress restores funding or extends flexibilities. Expect payment delays. 🔹 Telehealth Services- Many temporary Medicare telehealth flexibilities expired on October 1, 2025. Without congressional action, providers may need to consider Advance Beneficiary Notices (ABNs) for telehealth services not covered under permanent law. 🔹 Essential Services Continue- Medicare and Medicaid payments are expected to continue, since these programs are not tied to annual appropriations. However, processing may be delayed due to reduced staffing. 🔹 Non-Essential Activities Paused- CMS is scaling back non-critical activities (e.g., some surveys, oversight, and policy development). If you are waiting on a Medicare survey, expect delays. We’re monitoring CMS and congressional updates closely and will keep our clients informed. Health Insurance Carrier Lines of Business: Do They Want Healthcare Costs Up or Down? It Depends. Health insurance carriers have multiple lines of business. Whether or not the health insurance company wants healthcare costs to go up or down for a member depends on the line of business. For some lines of business, the health insurance carrier makes more money when healthcare costs go up. For other lines of business, the health insurance company makes more money when healthcare costs go down. Lines of Business: 1) Self-Funded Employer-Sponsored Health Plans: Want healthcare costs to go up. 2) Fully-Insured Health Plans: Want healthcare costs to go up. 3) Medicare Advantage: Want healthcare costs to go down. 4) Medicaid Managed Care: Want healthcare costs to go down. 5) Affordable Care Act (ACA) Plans: Want healthcare costs to go down. 6) Medicare Part D Prescription Plans: Want Healthcare costs to go down. Corollary: Health insurance carriers are incentivized to have stricter Prior Authorization to control healthcare costs for lines of business where they want healthcare costs to go down... i.e. Medicare Advantage, Medicaid Managed Care, ACA and Medicare Part D plans. In 2023, healthcare providers faced a staggering $25.7 billion in claims adjudication costs—a 23% increase from the previous year. Alarmingly, over 50% of these denials were ultimately overturned and paid, indicating that nearly $18 billion was potentially wasted on disputes over claims that should have been approved initially.   This inefficiency not only strains providers’ financial viability but also diverts critical resources away from patient care. The lack of a unified claims submission system, coupled with each payer’s unique rules, creates a labyrinthine process prone to errors and delays. Moreover, the largely manual nature of claims submissions exacerbates the problem, especially amid widespread staffing shortages.     It’s imperative that we bridge the gap between payers and providers by optimizing resource utilization, streamlining administrative processes, ensuring accurate reimbursement and ultimately improving patient outcomes. By addressing these systemic issues, we can redirect valuable resources back to where they belong — enhancing patient care and advancing the health of our communities.
