80% of companies are encountering risky AI agent behaviors. Only 24% have formal governance frameworks in place. That gap is a board-level crisis. McKinsey's latest research shows organizations are deploying AI agents faster than they can secure them. The misconduct categories are what you'd expect: improper data exposure, unauthorized system access, unintended actions. But here's what caught my attention - OpenAI's CISO publicly called prompt injection a "frontier, unsolved security problem." Think about that. The leading AI company admits they can't engineer their way out of this. Meanwhile, 48% of Fortune 100 boards now cite AI oversight - triple the rate from last year. But governance is lagging badly. We're seeing awareness without action. THREE QUESTIONS FOR LEADERS: - Should you slow down AI agent deployments until governance catches up, or is that competitive suicide? - If prompt injection is architecturally unsolvable, what does that mean for how you design agent permissions and monitoring? - When 67% of IT leaders say AI adoption is outpacing their understanding, how do you build trust with boards that are asking harder questions? Harvard Law's analysis shows this is hitting every sector. Financial services went from 14 to 63 companies disclosing AI risk in just two years.
