I’m glad to see a panel addressing the continuum of care and care coordination. As a Parkinson’s caregiver - outside of my work as a marketing executive - I can’t overstate how disconnected care teams often are. It’s not just a lack of comprehensive care. It’s also a reflection of how current payer models are structured. As your team noted, there’s an urgent need to build more localized, community-based care networks that can close these gaps and reduce fragmentation.
