We often think of asthma as a problem of airway tightening — but for many patients, mucous plugging is an equally powerful driver of symptoms and even severe attacks. When inflammation flares in the airways, mucus glands go into overdrive. The mucus thickens, becoming sticky and harder to clear. In some cases, it forms “plugs” that block airflow completely — trapping air and causing hypoxia, cough, or even life-threatening exacerbations. What’s behind it? Eosinophilic inflammation plays a key role. Elevated eosinophils release proteins that damage airway tissue and increase mucus production. In ENT practice, we often focus on the nasal and sinus benefits of antihistamines and decongestants - relief from congestion, sneezing and allergic inflammation. But one side effect that tends to go unnoticed is ocular dryness. By reducing tear film secretion, these medications can cause dry eye symptoms such as burning, grittiness or blurred vision. For patients already using them chronically for allergic rhinitis or sinusitis, this effect can significantly impact comfort and adherence. As ENTs, we are well-positioned to identify these symptoms early. A simple question about eye discomfort during review can make a difference in patient outcomes. It may also be time to revisit our prescribing approach for patients with coexisting sinus and ocular surface issues - choosing second-generation antihistamines or optimizing hydration can help maintain comfort while controlling allergy symptoms. 💡 ENT and eye health often intersect in subtle ways. Eczema is a common skin condition that affects people of every race and ethnicity. However, its appearance can vary depending on a person’s skin pigmentation. Symptoms are often more severe among people with skin of color, especially children. Eczema on black and brown skin, as well as other darker skin tones, often looks different compared to white skin. Some doctors and patients may mistake eczema on black skin and brown skin for other conditions. This can lead to delays in diagnosis and treatment. Social and economic factors can influence how and when a person receives an eczema diagnosis and impact eczema treatment. Ongoing challenges for people with skin of color include: • fewer providers who know how eczema appears on darker skin pigments; • providers that may underestimate eczema severity with darker skin pigments; • limited access to quality healthcare, including medical therapies. When Allergies and Autoimmunity Collide 🌬️ A new study in BMC Pulmonary Medicine found that people with Sjögren’s who develop chronic rhinosinusitis (CRS) have nearly a 50% higher risk of interstitial lung disease (ILD) than those without sinus involvement. It’s more evidence that the airway acts as one system — what happens in the sinuses doesn’t stay in the sinuses. We’ve long used the “one-airway, one-disease” model for allergic rhinitis and asthma. This research suggests the same may be true for autoimmune disease. When mucosal inflammation shows up in one part of the airway, it may signal risk deeper in the lungs. At the Immune Confident Institute, about 50% of our patients live with Sjögren’s. Based on this data, we’re updating our care pathways to proritize proactive airway evaluation and pulmonary function testing for those at higher risk. Allergy and autoimmunity aren’t separate categories — they’re connected points on the same immune spectrum. TWO CME COURSES PREPARE HEALTH CARE PROVIDERS TO HELP PATIENTS WITH ALPHA-GAL SYNDROME: There are an estimated 450,000 people suffering from alpha-gal syndrome in the United States. Alpha-gal syndrome (AGS) is a severe, life-threatening allergy that can develop after a tick bite. Also known as the “red-meat allergy,” AGS typically involves adverse reactions to mammalian products, which can be found in meat, dairy, medical supplies, makeup, and many household supplies. The symptoms are often delayed, making AGS more difficult to diagnose, and can be life-threatening, as in cases of anaphylaxis. With suspected cases rising sharply across the U.S. and many going undiagnosed, clinician awareness is urgently needed. Two new continuing medical education (CME) courses aim to change that by giving health care providers the knowledge they need to recognize AGS and help patients manage it — one by the CDC and one by VectorWise CME and Project Lyme, which is taught by Dr. Scott Commins, Associate Professor at the UNC School of Medicine and leading expert on alpha-gal syndrome.
