The American Academy of Pain Medicine has released a new international, multispecialty guideline on sacroiliac joint (SIJ) complex pain. Clinicians now have a unified, evidence-based framework for diagnosing and managing SIJ complex pain, endorsed by 27 professional organizations. The guideline clarifies what works, where evidence is limited, and where stronger research and policy alignment are urgently needed. AAPM, in partnership with the Pacific Northwest Evidence-based Practice Center at Oregon Health & Science University, has released the Draft Clinical Practice Guideline and Draft Systematic Review for public comment. We are seeking feedback from primary care, emergency medicine, and pain medicine professionals to ensure these recommendations are clear and relevant to real clinical settings. Medicare’s proposed coverage changes for peripheral nerve blocks would severely limit access to evidence-based, non-opioid pain care. On November 7, the American Academy of Pain Medicine submitted formal comments to all five Medicare Administrative Contractors, urging CMS to withdraw the proposed Local Coverage Determination (LCD) and align policy with contemporary science and patient need. Our advocacy reflects collaboration across national pain societies, federal agencies, and organized medicine to ensure equitable, patient-centered care remains protected.
