Our latest whitepaper highlights how PCP use of Counterpart Assistant (CA) technology is associated with earlier diagnosis and less frequent hospitalization in underserved populations. Key highlights from the whitepaper include: Higher Diagnosis Rates: Patients from socioeconomically disadvantaged neighborhoods attributed to a CA PCP who joined Clover Health from another MA plan were more likely to receive their first known diagnosis of diabetes, CKD, CHF, or COPD in their first year after plan enrollment. Diagnosis rates were 75% higher for diabetes; 89% higher for CKD; 89% higher for CHF; and 70% higher for COPD. Diagnosis at Earlier Stages: Patients from this population with CKD or diabetes were diagnosed at earlier clinical stages of disease. For CKD patients, initial clinical markers reflected CKD Stage 2, on average, versus Stage 3A. For diabetes patients, A1C levels were indicative of an earlier stage of disease with average A1Cs at 6.8% compared to 7.1% in the comparison group. Less Frequent Acute Care Utilization: CA PCP attribution was associated with significantly less acute care utilization, including fewer all-cause inpatient hospitalizations (ranging from -7.6% to -21.2%) and 30-day readmissions (ranging from -11.5% to -20.8%) across all four studied chronic diseases. We’re proud to share our RFI response and recommendations, and welcome the dialogue with the leadership at CMS, the Office of the National Coordinator, and the Assistant Secretary for Technology Policy. We highlighted five key areas to advance healthcare data exchange: 1️⃣ 𝗜𝗺𝗽𝗿𝗼𝘃𝗶𝗻𝗴 𝗗𝗮𝘁𝗮 𝗔𝗰𝗰𝗲𝘀𝘀 | We advocate for: a) a national FHIR-based standard for appointment scheduling as it plays a critical role in coordinating timely care and patient engagement, b) expansion of clinical data access via CMS’s Blue Button 2.0 to empower beneficiaries and their caregivers to meaningfully engage in their care and boost providers’ ability to identify care gaps and improve care coordination, c) the structured exchange of imaging and diagnostic data to decrease delays in clinical decision-making and enhance the ability of health-technology platforms to support informed care delivery. 2️⃣ 𝗥𝗲𝗱𝘂𝗰𝗶𝗻𝗴 𝗗𝘂𝗽𝗹𝗶𝗰𝗮𝘁𝗶𝗼𝗻 & 𝗦𝘁𝗿𝗲𝗻𝗴𝘁𝗵𝗲𝗻𝗶𝗻𝗴 𝗣𝗿𝗼𝘃𝗲𝗻𝗮𝗻𝗰𝗲 | We urge CMS and ONC to integrate detailed requirements for provenance tagging and data reuse into the ONC Health IT Certification Program to ensure data accuracy and reliability. 3️⃣ 𝗦𝘁𝗿𝗲𝗻𝗴𝘁𝗵𝗲𝗻𝗶𝗻𝗴 𝗧𝗘𝗙𝗖𝗔 & 𝗣𝗼𝗹𝗶𝗰𝘆 𝗔𝗹𝗶𝗴𝗻𝗺𝗲𝗻𝘁 | TEFCA is the right mechanism behind which we can continue to expand. We shared 7 specific policy recommendations to enhance the frameworks in place, including creating paved pathways for treatment and healthcare operations requests, harmonizing local and national privacy laws, and expanding API access to extend to all current and past members. 4️⃣ 𝗘𝗺𝗽𝗼𝘄𝗲𝗿𝗶𝗻𝗴 𝗣𝗮𝘁𝗶𝗲𝗻𝘁𝘀 | We propose the use of NIST 800-63-3 IAL2 authenticated requests for all Patient Access API calls to standardize digital identity credentials and improve access for all beneficiaries. 5️⃣ 𝗖𝗹𝗮𝗿𝗶𝗳𝘆𝗶𝗻𝗴 𝗜𝗻𝗳𝗼𝗿𝗺𝗮𝘁𝗶𝗼𝗻 𝗕𝗹𝗼𝗰𝗸𝗶𝗻𝗴 𝗥𝘂𝗹𝗲 𝗳𝗼𝗿 𝗩𝗲𝗻𝗱𝗼𝗿𝘀 | We recommend removing unnecessary barriers and improving interoperability by clarifying and facilitating business associate access to vendor-held data on behalf of a covered entity. We believe these steps will significantly improve the health information ecosystem, benefiting providers and, most importantly, patients. Our latest data report highlights how a relationship with a PCP that uses Counterpart Assistant (CA) is associated with better health outcomes in patients with congestive heart failure (CHF). Our analysis demonstrates that a relationship with a PCP that uses CA is associated with better clinical care for heart failure patients enrolled in Clover Health’s Medicare Advantage (MA) plans, including higher rates of outpatient cardiologist visits and in-home care management. In turn, a relationship with a CA PCP is also associated with better outcomes, including a lower average number of all-cause hospitalizations (18% lower) and 30-day readmissions (25% lower). CA continues to provide actionable insights at the point of care, which supports earlier detection of chronic disease, tracking of disease progression and severity, and guideline-based treatment.
