Community Health Centers (CHCs) serve at least 1 in 10 Americans yet account for just 1% of U.S. health care spending—proving that primary care saves lives AND lowers costs. As the nation’s largest primary care network, CHCs are on the front lines of fighting chronic disease. In 2024 alone: ✅ 3.5M patients received diabetes care—72% achieved A1c control ✅ 6.2M treated for hypertension—67% reached blood pressure control ✅ 10M received obesity care—72% of youth got screened & counseled on nutrition and activity CHCs also reach beyond exam rooms with services like nutrition counseling, fitness and cooking classes, peer support, and career counseling—helping patients stay healthy while reducing overall costs. 👉 With greater investment, CHCs can close primary care gaps and help reverse the nation’s chronic disease crisis. Dr. Jack Geiger, one of the founders of the earliest Community Health Centers, recognized that patient-directed care is the CHC special sauce. NACHC mourns the passing of Sue Veer, President and CEO of Carolina Health Centers, Inc., a visionary national leader for community health, and a longstanding member of the Executive Committee of the NACHC Board of Directors.
