Neighborhood Health Center's President & CEO Joanne Haefner (pictured far right) joined leaders from four other local non-profits and Univera Healthcare President Paul Valley on WIVB's Daytime Buffalo program. They were there for the announcement of this year's Univera Healthcare Health Equity Innovation Awards. We are grateful to receive an award this year to help fund our “Bridging Health Outcome Gaps through Social Needs Screening and Response” initiative. Neighborhood has found success in reducing disparities in low birthweight deliveries in part through social needs screenings for pregnant patients, and connecting patients to medical and social resources they need for healthy pregnancies. The 2025 award from Univera Healthcare will enable us to replicate this process for patients with hypertension and diabetes. Thank you, Univera Healthcare!

Welcome to Neighborhood, Grace Bachner, FNP-C! Grace is a nurse practitioner in Neighborhood Health Center's internal/family medicine department. She earned a Master of Science degree in Advanced Practice Nursing at St. John Fisher University, after earning her undergraduate degree in Nursing at the University at Buffalo. We spent the last couple of months renovating Neighborhood Health Center Southtowns (151 Elmview Ave.) so we could expand services and better serve you. New patients are welcome, and no one is turned away for inability to pay. We offer a sliding fee program for those who are uninsured or under-insured. Thank you Pat, Shilpa, and Rafay! The second year medical students from the Jacobs School of Medicine and Biomedical Sciences at the University at Buffalo are part of the Community Connect service learning program. It's designed to teach UB medical students about the community and the social forces that impact patients' lives. This year they've spent time at Neighborhood Health Center learning about the patients we care for, and the community. They've conducted social needs screenings for high risk patients, learned how these screenings inform patient care, and shadowed community health workers in connecting patients to community resources. We've enjoyed having you here Pat, Shilpa, and Rafay, and we hope you've found the experience beneficial.

Neighborhood Health Center is expanding to North Buffalo! We're taking over a vacant former urgent care center at 2497 Delaware Avenue, just north of Hertel. We'll be doing some renovations with a goal of having it ready to start seeing primary care patients in late spring 2026. This will be the seventh Neighborhood Health Center location in WNY. As the region's top ranked health center for quality, we're excited to be able to expand to meet the primary care needs of our beloved community members.
